Public health measures like isolation, quarantine, and mask-wearing used to control infectious diseases in the absence of vaccines or treatments.

Non-pharmaceutical interventions (NPIs) are strategies used to prevent or control the spread of infectious diseases without using medications or vaccines. These are actions that individuals, communities, or governments can take to reduce transmission, especially when vaccines or treatments are unavailable, limited, or only partially effective. NPIs can range from individual behaviors, e.g., handwashing, wearing masks, or staying home when sick, to population-level policies, e.g., school closures, travel restrictions, or bans on mass gatherings.

NPIs are the first line of defense against newly emerging infections and remain critical even after vaccines or treatments become available. In the early phase of an outbreak, especially with novel pathogens like COVID-19, NPIs can slow the spread of infection, protect healthcare systems from being overwhelmed, and buy time for vaccine development and distribution.

NPIs are also essential for controlling diseases where vaccines don’t exist (like norovirus), are only partially protective (like influenza), or where antimicrobial resistance threatens treatment effectiveness. The success of NPIs depends heavily on community adherence, clear public communication, trust in public health authorities, and the social and economic ability of people to comply.

Types of Non-Pharmaceutical Interventions (NPIs)

NPIs operate at both individual and community levels to prevent infection and limit transmission.

Individual-level NPIs

Personal behaviors that reduce the risk of infection and transmission.

  • Hand hygiene (frequent handwashing or hand sanitizer use)
  • Respiratory etiquette (covering coughs and sneezes)
  • Wearing masks in public spaces
  • Staying home when sick
  • Physical distancing from non-household members
  • Cleaning and disinfecting frequently touched surfaces

Community or policy-level NPIs

  • School and workplace closures or modified operations
  • Cancellation of mass gatherings and events
  • Travel restrictions or border controls
  • Stay-at-home orders or lockdowns
  • Capacity limits in indoor public spaces

Implementing NPIs effectively requires balancing public health goals with social, economic, and educational considerations.
