Covid-Zero Cracks in Hong Kong: What Went Wrong?

Once heralded as a model for pandemic control, Hong Kong’s approach—centered on total elimination rather than long-term coexistence—was proving unsustainable in the face of Omicron’s rapid transmissibility.

“Elimination strategies work when you can completely seal off borders and have high vaccination uptake, particularly among older populations,” Varma explained. “But Hong Kong didn’t vaccinate its elderly population at sufficient levels.”

This, combined with Omicron’s stealthy spread and gaps in health system preparedness, led to record infections and overwhelmed hospitals, challenging the viability of similar policies in mainland China.

China’s Dilemma: Elimination vs. Endurance

Varma warned that China faces similar risks if it continues down a zero-COVID path without evolving its strategy. While China’s vast testing infrastructure and centralized control offered short-term benefits, he noted that the virus was outpacing the policy.

“You can’t hold the virus at bay forever,” Varma said. “Eventually, highly transmissible variants will exploit even the smallest breach.”

He recommended that policymakers prioritize vaccine campaigns—particularly for vulnerable groups, improve booster uptake, and begin transitioning toward policies that mitigate severe outcomes rather than aim for total suppression.

War and Viruses: The Ukraine Factor

In a striking pivot, Dr. Varma also discussed how the war in Ukraine could aggravate the global pandemic response. The conflict’s displacement of millions and diversion of resources raised alarms about surging infections, broken supply chains, and lost disease surveillance capacity across Europe and beyond.

“Wars disrupt everything,” he cautioned. “When health systems collapse and populations are on the move, infectious diseases thrive.”

He urged global leaders to maintain pandemic funding and vigilance, even amid geopolitical instability, stressing that crises do not occur in isolation.

Final Thoughts: Adaptability is the New Gold Standard

Varma concluded with a strong call to abandon rigid ideology in pandemic policy. Whether in China, Europe, or the U.S., he emphasized that governments must be agile—able to shift gears from suppression to mitigation, from lockdowns to layered protections.

“The playbook must evolve,” he said. “What worked in 2020 won’t work in 2022.”
